Wish Application Form

To apply for a Wish from Make-A-Wish Australia, you need to submit an application to the Make-A-Wish National Office. Children may fill out the form themselves but it must be signed by a parent or legal guardian.

Alternatively, ask for a Wish application form at your local hospital or support group, phone Make-A-Wish on 1800 032 260, or send us an email at wishes@makeawish.org.au and we will send an application form out to you.

All applicants will be notified by phone once the form has been received.
